CSAB Fellow Nomination Kit

This kit is designed to help you in nominating a peer for recognition as CSAB Fellow. In it you will find:

• CSAB FELLOW NOMINATION GUIDE
• CSAB FELLOW NOMINATION FORM
• CSAB FELLOW REFERENCE FORM

Please review the enclosed material carefully and proceed as follows:

1. Assure yourself that the candidate is worthy of consideration for this prestigious award.
2. Contact three to five persons to determine whether they will be willing and eligible to complete a reference form. Please refer to Section 5l, of the Nomination Guide for special requirements.
3. Send via email a copy of the CSAB FELLOW REFERENCE FORM to each person who has agreed to be a reference for the candidate. As an alternative, you can provide the URL of the CSAB website from which the reference form can be obtained. Reference forms are to be submitted directly to CSAB.
4. Submit the completed CSAB FELLOW NOMINATION FORM via email to csab@csab.org. Keep a copy of the all forms for your records.
5. The Nomination Form and references must be received at CSAB Headquarters by the last Friday in May.
6. Expect an email confirmation of receipt from CSAB Headquarters. If a confirmation is not received within one week, please contact CSAB Headquarters via email or telephone (203-975-1117).
7. The nominee should expect notification of the outcome of a vote by the CSAB Board of Directors before the end of September.
